Who Is a Good Candidate for IVF?

Couples that have been unsuccessful in getting pregnant with other fertility treatments often turn to IVF to conceive. Women with tubal issues can benefit from IVF. Since the IVF procedure bypasses the fallopian tubes entirely, many women with blocked or absent fallopian tubes find that IVF helps them to conceive. Other good IVF candidates include women with fertility disorders like endometriosis and polycystic ovary syndrome (PCOS). IVF can help them overcome their infertility symptoms and become pregnant. Women with abnormal ovulation cycles can also get pregnant with IVF, since fertility drugs can be used to induce ovulation and generate healthy eggs.

Who May Not Benefit from IVF?

In vitro fertilization (IVF) isn’t for everyone. If you do not want to use donor eggs during IVF, and your ovaries do not produce healthy eggs, you will have little success with IVF.

Also, since egg production slows as a woman approaches age 40, women older than 37 may have a reduced chance of getting pregnant with IVF.

If you are past your late 30s, you may consider using donor eggs to increase your chances of conceiving with IVF. Use of donor eggs with IVF achieves similar pregnancy rates for women of all ages, and about 50 percent of women that attempt IVF with donor eggs are able to get pregnant. However, even if older women do get pregnant with donor eggs, women over 40 still have a greater chance of miscarriage.

Aging and Fertility

Decline in live birth success rates by female age beginning at approximately age 31. Infertility increases with age. This curve becomes steeper starting at about age 37. At age 44 and above births resulting from IVF using the female’s own eggs are almost nonexistent. The live birth success rate using eggs over age 43 is 2% per attempt. Most IVF centers are willing to attempt IVF using the female partner’s eggs until about age 43-45, after which the couple would be offered egg donation as the only realistic and appropriate fertility approach.

A study published in 1957 by Tietze in Fertility and Sterility examined the relationship between the age of the female partner and fertility. This study found that the percentages of infertile couples were:

IVF Risks Include:

Multiple births. IVF increases the risk of multiple births if more than one embryo is implanted in your uterus. A pregnancy with multiple fetuses carries a higher risk of early labor and low birth weight than pregnancy with a single fetus does.
Premature delivery and low birth weight. Research suggests that use of IVF slightly increases the risk that a baby will be born early or with a low birth weight.
Ovarian hyper stimulation syndrome.Use of injectable fertility drugs, such as human chorionic gonadotropin (HCG), to induce ovulation can cause ovarian hyper stimulation syndrome, in which your ovaries become swollen and painful.
Miscarriage. The rate of miscarriage for women who conceive using IVF with fresh embryos is similar to that of women who conceive naturally — about 15 to 25 percent — but the rate increases with maternal age. Use of frozen embryos during IVF, however, may slightly increase the risk of miscarriage.
Egg-retrieval procedure complications. Use of an aspirating needle to collect eggs could possibly cause bleeding, infection or damage to the bowel, bladder or a blood vessel. Risks are also associated with general anesthesia, if used.
Ectopic pregnancy.About 2 to 5 percent of women who use IVF will have an ectopic pregnancy. when the fertilized egg implants outside the uterus, usually in a fallopian tube, The fertilized egg can’t survive outside the uterus and there’s no way to continue the pregnancy.
Birth defects.The age of the mother is the primary risk factor in the development of birth.
